Caring for your child

The goal of this form is to collect and store pertinent information regarding the care of your child for the use of the Kids Ministry Team. Our mission is to take everyone we meet one step closer to being a true disciple of Jesus Christ. We desire to give parents respite by freeing them to fully participate in the Sunday morning service without needing to care for their child. We also desire to take your child one step closer in being a true disciple of Christ by instructing and loving them in Christ-centered, developmentally-appropriate ways. By going through this form with you, we hope to have enough knowledge to provide a safe, welcoming, and fun environment for your child!

Please note room placement will be decided not by age but by collaboration of parents and special needs team, in order to place child in an environment that is the most supportive and accessible to their needs.

If your child is in a diaper and needs assistance past the age of 4 we will call you to assist in bathroom procedures.

SAFETY: Is your child a flight risk? Can they open doors independently?

SAFETY: How does your child move around the room? (ie. supportive devices, awareness of personal space)

SAFETY: Is there anything else we should be aware of to keep your child safe?

FUN: Are there any toys or activities that are favorites of your child? (Or that they do not like!)
